Job summary

Bristol Hospice is seeking an Admissions RN to establish and maintain positive relationships with community and referral sources, while expanding access to hospice services in the service area. The role involves coordinating admissions and educating patients and families on the plan of care.

Ideal candidates hold an Alabama RN license with at least 1 year of nursing experience and are willing to travel up to 20% of the time, often with little notice.

Qualifications

• Must hold an active RN license in Alabama.
• Minimum 1 year of professional nursing experience within last 3 years or BSN from NLN-accredited program.
• Ability to travel 20% of the time, often without notice.

Responsibilities

• Establish and maintain relationships with communities, referral sources, and DPCS/ED.
• Coordinate admissions and hospice referrals to promote timely care.
• Provide patient/family education about the plan of care and hospice services.
• Collaborate with ED, IDT, and intake to ensure smooth patient transitions.
