Job summary

Bristol Hospice is seeking an Admissions RN to build relationships with customers and referral sources and to expand hospice access in the communities it serves. The role involves coordinating admissions, completing referrals, and providing nursing care per the plan of care across patient homes.

The ideal candidate will hold an active RN license, a BSN preferred, and be willing to travel about 20% with flexible hours.

Qualifications

• Must have a current RN license in the state.
• Minimum of 1 year nursing experience or a BSN from an NLN-accredited program.
• Willingness to travel ~20% and flexible hours.
• Ability to work autonomously with strong communication skills.

Responsibilities

• Establish and maintain relationships with customers and referral sources.
• Coordinate admissions and timely admission processes for hospice care.
• Complete hospice consults/evaluations and admissions.
• Provide professional nursing care per the plan of care.
• Educate patients and families about hospice services and care options.
