Care Initiatives Hospice is seeking a compassionate Hospice Admissions Nurse to join our team and facilitate patient transitions of care during admission. The role involves meeting patients and families where they are most comfortable — at home, in long-term care, assisted living, or hospital — and coordinating with Central Intake, Clinical Manager, Program Director, and Community Representative.

As an RN with hospice or admissions experience, you will provide visit support for new admissions

Qualifications

• Current, valid Iowa registered nurse license or license in a compact state.
• Two (2) years recent hospice or home care experience or admissions role in medical/surgical, oncology, or facility nursing.
• Current, valid CPR certification.

Responsabilités

• Provide nursing services to Hospice Initiatives Hospice patients through visits, including telephonic and in-person management of patients, families, caregivers and referral sources.
• Establish positive relationships with community sources to enhance admissions and coordinate care, medications, equipment, and supplies.
• Determine immediate needs and initial assessment, share findings with the RN-Case Manager and collaborate care with the IDT.
• Provide visits to assess ongoing patients’ needs, revise care plans, and communicate assessments to the team when not performing admissions.
• Provide clinical services addressing goals of the care plan.
• Prepare clinical and progress notes; maintain records; coordinate services and inform physician and staff of changes.
• Communicate with staff and resources to coordinate services for patients and families.
