Job summary

Chapters Health System seeks an Admission RN to assess patients at home or hospital settings and coordinate seamless transitions to Care Navigation. This role requires Florida RN licensure and at least 1 year of nursing experience, with hospice/hospital exposure preferred.

You will communicate effectively in English, with Spanish bilingualism preferred, and must hold a valid driver’s license to travel between facilities. A reliable vehicle and strong customer service are essential.

Qualifications

• Current RN license in the state where employed.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR (Electronic Medical/Health Record) systems.
• Strong clinical assessment skills; emergency department or ICU background preferred.
• Ability to communicate in English (verbal and written); bilingual in Spanish preferred.
• Mobile driver with valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet schedule.
• Ability to work in a team environment and handle multiple priorities under stress.
• Ability to perform essential functions and physical requirements with or without reasonable accommodation.
• Excellent customer service focus.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
