Job summary
Chapters Health System seeks an Admission RN to assess patients in their residence or hospital and initiate care with our services. The role acts as a liaison to Care Navigation and represents the organization across facilities.
The ideal candidate holds an RN license in Florida, has at least 1 year of nursing experience (hospice/hospital preferred), and is proficient with EMR/EHR systems. Spanish language skills are a plus, and a valid driver’s license with reliable transportation is required.
Qualifications
- Current RN license in the state of employment.
- Minimum of one year nursing experience; hospice or hospital preferred.
- Experience with EMR/EHR systems.
- Strong clinical assessment skills; ED/ICU background a plus.
- Ability to communicate in English; Spanish a plus.
- Valid driver’s license and reliable transportation.
Responsibilities
- Complete clinical admissions for all company services.
- Explain services to patients/families and address questions empathetically.
- Initiate Plan of Care and coordinate with physicians.
- Perform skilled nursing interventions to manage symptoms and comfort.
- Maintain timely documentation and liaison with care navigation.
