Qualifications

• RN licensure in Maryland (Current)
• Two years of nursing experience or equivalent education/experience
• Education: Diploma, ADN and/or BSN from an accredited institution

Responsibilities

• Perform initial and ongoing patient/family assessments in home care to determine care needs
• Develop and implement care plans with the Interdisciplinary Group
• Provide direct nursing care to terminally ill patients in home or facility settings
• Supervise delivery of care by HHAs and coordinate with team members
• Document care using facility EMR and communicate with physicians and families
• Educate family members and caregivers on medications, procedures, and symptom management
• Ensure equipment arrangements and safety in home or facility settings
• Attend care plan meetings and update care teams regularly
