Job summary

GBMC HealthCare in Towson, MD seeks a Hospice Admissions Nurse to assess patients in their home or hospital and prepare entry into Gilchrist Hospice Care services. This role also represents GBMC HealthCare as an ambassador in the community.

You will complete initial evaluations, develop care plans with Medical Director approval, provide direct nursing care, educate patients and families, and collaborate with care teams while upholding GBMC values.

Qualifications

• ADN and/or BSN from an accredited institution.
• Current MD nursing license.

Responsibilities

• Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs.
• Prepares initial care plan for patient/family with Medical Director approval.
• Provides direct nursing care to maintain highest level of care.
• Educates family members or hired caregivers on patient care skills and medications.
