Job summary

GBMC HealthCare is seeking a Hospice Admissions Nurse who assesses patients in their home or hospital settings and coordinates entry into Gilchrist Hospice Care services. The role also acts as an ambassador for GBMC HealthCare within the service area.

Responsibilities include completing initial assessments, care planning with Medical Director approval, providing direct nursing care, collaborating with teams, educating patients and families, and maintaining relationships with referral sources in

Qualifications

Diploma, ADN and/or BSN from an accredited institution.
Current MD nursing license.
Three years of nursing experience; home health or hospice preferred.

Responsibilities

Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs.
Prepares and implements the initial care plan for patient/family unit with Medical Director approval.
Provides direct nursing care to patients to maintain highest level of care.
Educates family members or hired caregivers on patient care skills and medications.
Documents findings to ensure hospice eligibility and participate in discharge planning.
Establishes and maintains relationships with hospice referral sources in the service area.
