Job summary

GBMC HealthCare's Gilchrist Inova team seeks a Hospice Admissions Nurse to be the compassionate first connection for patients entering hospice care. You will complete admissions, provide education, and coordinate with the care team to ensure a smooth transition.

Ideal candidates are RNs with a diploma/ADN/BSN and at least three years of nursing experience. Virginia RN licensure and strong communication are required. Mileage reimbursement and competitive pay are offered.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Three years of nursing experience; Home Health or Hospice preferred but not required.
• Strong interpersonal and communication skills; patient/family education.

Responsibilities

• Completes initial patient assessment for hospice eligibility and plans care with Medical Director approval.
• Provides direct nursing care to patients in home, hospital, or facility settings.
• Coordinates with the interdisciplinary care team and external agencies as needed.
• Educates patients and families on care plans, medications, and symptom management.
• Documents clinical findings and maintains compliance with regulations.
