Job summary

Amedisys, part of the Optum family, is hiring a Hospice Admissions RN to guide patients and families through the admission process, assess needs, and coordinate an individualized plan of care. You will collaborate with physicians and the interdisciplinary team to ensure timely, compassionate care.

You’ll provide skilled interventions, document progress, and function as a liaison while traveling between facilities as required.

Qualifications

• Current RN license valid for the assigned state(s).
• CPR certification is required.
• 2+ years of clinical RN experience in acute care settings.
• Valid driver's license and reliable transportation.
• Proficiency with EMR systems preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Coordinate hospice admissions by assessing initial physical, psychosocial and spiritual needs.
• Develop the initial plan of care with the attending physician and team.
• Provide interventions and caregiver guidance for symptom control and comfort.
• Serve as liaison between disciplines and facilities during admission.
• Document care progress accurately and timely per policy.
• Report significant changes to the clinical manager and physician.
• Fulfill on-call duties and other assigned tasks.
