Job summary

UnitedHealth Group's Amedisys hospice admissions team is seeking a registered nurse to guide patients and families through admission, assess needs, and coordinate an individualized plan of care.

You will collaborate with physicians, facilities, and the interdisciplinary team, perform on-call duties, and provide skilled interventions and education to support comfort and symptom control while maintaining thorough documentation.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• Current CPR certification.
• 2+ years of clinical RN experience in acute healthcare settings.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients' initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies.
• Communicates significant findings and changes in condition to the clinical manager/physician and other hospice care team members.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
