Job summary

Affinity Hospice Management LLC is seeking an Admission RN to plan, organize, and direct hospice care with emphasis on community health. The RN works within patients' homes and communities, building from local resources to meet patient and family needs.

The role involves completing comprehensive patient assessments, developing care plans, administering prescribed treatments, educating patients and families, and coordinating care as Case Manager when assigned.

Qualifications

• Experience nursing with emphasis on community health or hospice care.
• Provides professional nursing care using the nursing process.
• Coordinates patient care as Case Manager when assigned.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family.
• Provide professional nursing care using the nursing process.
• Develop and revise plan of care with patient and family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient and family in nursing and related needs.
• Coordinate patient care and act as Case Manager when assigned.
• Document and communicate patient conditions with the physician.
• Maintain safe environment and continuity of care.
• Arrange equipment and services as needed.
• Engage in quality improvement activities.
