Qualifications

• Current, unrestricted RN license in MA; hospice/home health experience preferred.
• Proficient in patient assessment, care planning, and symptom management in hospice/palliative care.
• Strong communication and ability to educate patients, families, and caregivers; coordinate with interdisciplinary teams.

Responsibilities

• Coordinate and complete hospice admissions for eligible patients.
• Conduct comprehensive nursing assessments and document findings.
• Explain hospice services and philosophy to patients and families; obtain consents and documentation.
• Collaborate with physicians, clinical staff, and interdisciplinary team to develop initial plans of care.
• Document clinical encounters accurately in the EMR; ensure smooth transition into hospice services.
• Educate patients and families; advocate and coordinate ongoing care post-admission.
