Job summary

Bristol Hospice, LLC in Greater Los Angeles, CA, seeks an Admissions Registered Nurse to join our full-time, on-site team. You will perform timely hospice admission assessments, explain services to patients and families, and obtain necessary consents and documentation.

You will collaborate with physicians and the interdisciplinary team to determine eligibility, start care plans, coordinate transitions from hospitals or facilities, ensure regulatory compliance, and educate families on medications

Qualifications

• Active RN license in California; hospice or palliative care experience preferred.
• Strong communication with patients, families, and the care team.
• Proficient documentation and adherence to regulatory standards.
• Organizational skills to manage multiple admissions and transitions.
• Travel locally as needed to patient homes and facilities.

Responsibilities

• Perform timely, comprehensive hospice admission assessments.
• Explain hospice services and care options to patients and families.
• Obtain consents and documentation necessary for admission.
• Coordinate start of services and initial care plans with the interdisciplinary team.
