Qualifications

• Must hold a state RN license.
• Two years of RN experience; hospice experience preferred.
• Current CPR and BCLS certification.
• Current driver's license required.

Responsibilities

• Collaborate with the Inter-Disciplinary Team to develop initial plan of care.
• Ensure initial patient needs are addressed and communicate essential information to the team.
