Job summary

Bristol Hospice is seeking an Admissions Registered Nurse in Riverside, CA to build relationships with referral sources and communities, and to increase access to hospice services in the service area. You will manage admissions, conduct initial assessments, and coordinate with hospital and facility teams to ensure timely, compassionate care.

The role requires an active CA RN license, 1+ year nursing experience or a BSN, and the ability to travel up to 20%.

Qualifications

• Must hold a current RN license in California.
• Minimum of 1 year nursing experience in the last 3 years or a BSN from an NLN-accredited program.
• Willingness to travel up to 20% and flexible hours.

Responsibilities

• Establish and maintain relationships with customers and referral sources in the community.
• Coordinate hospice admissions and complete initial assessments.
• Collaborate with ED, DPCS, and community education leaders to promote hospice access.
• Provide patient and family education on the plan of care and hospice services.
• Participate in IDT meetings and quality initiatives as directed.
