Job summary

Bristol Hospice - High Desert in Klamath Falls, OR, is seeking an Admissions RN to establish and maintain relationships with customers and referral sources within the Bristol Hospice network. You will develop access to hospice services in the communities served and coordinate admissions with hospital and facility teams.

The role requires an active RN license in the state, up to 20% travel, and flexibility in hours, with a focus on compassionate, patient-centered care and strong communication

Qualifications

• Must hold a current RN license in the state.
• Minimum of one year of professional nursing experience within the last three years or a BSN from an NLN-accredited program.
• Must be willing to travel up to 20% of the time.
• Must be flexible in work hours.
• Must maintain knowledge of information systems and applicable software.

Responsibilities

• Establish and maintain relationships with customers and referral sources to promote hospice access in the community.
• Develop and increase access to Bristol Hospice Services in the served communities.
• Coordinate admissions and ensure timely patient referrals and bed availability.
• Communicate with ED, DPCS, and other care teams to plan and evaluate patient plans of care.
• Provide professional nursing care using all elements of the nursing process and comprehensive assessments.
