Bristol Hospice Eastern Oklahoma, Fort Gibson, OK is seeking a full-time Admissions RN to establish and maintain relationships with referral sources and patients to expand access to hospice services. The role involves conducting admissions, coordinating with ED and DPCS, completing consultations, assessing patients, and ensuring timely admissions, while traveling up to 20% of the time.

Qualified candidates hold a current RN license in Oklahoma, with 1 year of nursing experience or a BSN, and

Qualifications

• Must have a current RN license in the state
• Minimum 1 year of experience as a professional nurse within the last 3 years or a bachelor’s degree in nursing from an NLN-accredited program
• Ability to travel ~20% of the time
• Flexibility in work hours and willingness to learn

Responsibilities

• Establish and maintain relationships with referral sources and community education directors
• Assess and meet the educational needs of the community related to hospice care
• Coordinate hospice referrals, admissions, and coordination of care
• Complete hospice consults/evaluations and admissions
• Communicate with referral sources and agency leadership to promote care coordination
