Job summary

Bristol Hospice in Eugene, OR is seeking a dedicated Admissions RN to establish relationships with community referral sources and expand access to hospice services across the Eugene area.

The role requires an active Oregon RN license, 20% travel, flexible hours, and commitment to hospice education. You will coordinate admissions, support care coordination with ED/DPCS, and provide compassionate care to patients and families.

Qualifications

• Must hold an active RN license in Oregon.
• Minimum 1 year nursing experience within last 3 years or a BSN from an NLN-accredited program.
• Ability to travel roughly 20% of the time, often with short notice.
• Flexibility in work hours to meet patient and care needs.
• Willingness to maintain knowledge of systems and software used in care coordination.

Responsibilities

• Relate information to DPCS, ED and Community Education Director to promote hospice access.
• Assess educational needs of the community related to hospice care and services.
• Establish and monitor perceptions of Bristol Hospice as a high-quality provider.
• Coordinate with agency and referral sources to promote care continuum.
• Complete hospice consults, evaluations and admissions.
• Coordinate with hospice leadership and referral sources for timely admissions.
• Provide professional nursing care using the nursing process.
• Participate in IDT meetings and educate patients and families.
• May participate in QA/QI activities and act as RN Case Manager as assigned.
