Job summary

Bristol Hospice - Georgia Llc seeks an Admissions RN to build relationships with customers and referral sources in the community and to expand access to hospice services within our Georgia locations.

Qualifications

• Must hold a current RN license in the state.
• Minimum of 1 year of professional nursing experience within the last 3 years or a BSN from an NLN-accredited program.

Responsibilities

• Establish and maintain relationships with customers and referral sources in the community.
• Promote Bristol Hospice services and ensure timely admissions and care coordination.
• Coordinate with ED, DPCS, and intake to facilitate admissions and education.
• Provide patient/family education and perform initial assessments to determine hospice needs.
• Possess flexibility in hours and travel up to 20% to support field-based care.
