Job summary

Your Talent Team is seeking a Hospice Admissions RN in the Kansas City Metro, MO (east). This full-time role covers a 50-mile radius from the office with a Tue-Sat, 10am-7pm schedule. You will coordinate admissions, assess patients, educate families, and partner with the care team to ensure smooth transitions into hospice services.

Requirements include MO RN license, current CPR, and hospice experience, plus strong communication and decision-making skills. Travel within the territory is required.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications
