Acloché LLC is seeking a Hospice Admissions RN for the Kansas City metro (east) area. Full-time role coordinating new patient admissions, completing assessments, educating patients and families, and partnering with the care team to ensure a smooth transition into hospice services.

Territory includes a 50-mile radius from the office with a Tue-Sat 10am-7pm schedule; candidates must be a Missouri-licensed RN, with hospice experience preferred.

Qualifikationen

• Missouri RN license in good standing.
• Current driver's license and CPR certification.
• Two years RN experience in a clinical setting.
• One year full-time hospice experience preferred.
• Reliable transportation and automobile liability insurance.

Hospice Admissions RN

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities

• Coordinate and oversee all aspects of new patient admissions
• Collaborate with patient care teams to support seamless communication and continuity of care
• Educate patients and families regarding condition, needs, and care
• Perform head-to-toe physical assessments
• Complete all required documentation accurately and on time
• Perform other duties as assigned
