ExcelliSearch, Inc. is seeking a dedicated Hospice Admissions Registered Nurse for the Kansas City Metro area.

The role focuses on coordinating new patient admissions, conducting comprehensive assessments, and educating patients and families while partnering with the interdisciplinary team to ensure a smooth transition into hospice services. Required qualifications include an active RN license in Missouri, CPR certification, a valid driver’s license, and hospice experience.

Qualifications

• Current Registered Nurse license valid for practice in Missouri or as required by state.
• Current driver’s license.
• Current CPR certification.
• Two years of RN experience in a clinical setting.
• One year of full-time hospice experience.
• Reliable transportation and automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to ensure seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri

• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
