Job summary

Hospice Admissions RN in the Kansas City Metro, MO (east) is a full-time role supporting Eden Capital Careers. You will coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and partner with the interdisciplinary care team to ensure a smooth transition into hospice services.

This position requires MO RN licensure, CPR, and hospice experience, plus a valid driver’s license.

Qualifications

• Missouri-licensed RN in good standing.
• Active CPR certification.
• Minimum two years RN clinical experience.
• One year hospice experience preferred.
• Valid driver’s license and reliable transportation.
• Ability to work independently and make clinical decisions.
• Strong written and verbal communication skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions
• Collaborate with patient care teams to support seamless communication and continuity of care
• Educate patients and families regarding condition, needs, and care
• Perform head-to-toe physical assessments
• Complete all required documentation accurately and on time
• Perform other duties as assigned

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities
