Job summary

Top Echelon Software, LLC Hospice provider seeks a full-time Admissions RN in the Kansas City Metro, MO (east) within a 50-mile radius from the office. The position involves coordinating new patient admissions, performing comprehensive assessments, educating patients and families, and partnering with the inter disciplinary care team to ensure a smooth transition into hospice services.

The schedule is Tuesday through Saturday, 10 a.m.

Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver\'s license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Responsibilities

• Coordinate and oversee all aspects of new patient admissions
• Collaborate with patient care teams to support seamless communication and continuity of care
• Educate patients and families regarding condition, needs, and care
• Perform head-to-toe physical assessments
• Complete all required documentation accurately and on time
• Perform other duties as assigned

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications
