Job summary

Day City Hospice is seeking a Registered Nurse (RN) Admissions Nurse in Dayton, OH. You will screen patients for hospice eligibility, educate families, facilitate admissions, and craft initial care plans to ensure comfort and quality of life.

The role emphasizes compassionate, patient- and family-centered care, with collaboration across an interdisciplinary team and a commitment to high-quality end-of-life services.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current RN license in the state of Ohio.
• Current CPR certification.
• Hospice experience preferred (1 year).
• Strong clinical judgment and ability to make independent decisions.
• Excellent communication and interpersonal skills.
• Ability to work compassionately and professionally with patients, families, caregivers, and healthcare professionals.
• Knowledge of quality assurance and quality improvement practices.

Responsibilities

• Conduct pre-admission evaluations to determine hospice eligibility and appropriateness of services.
• Educate patients and families about hospice care and available services.
• Facilitate the admission process and obtain informed consent.
• Perform comprehensive nursing assessments.
• Develop initial plans of care based on patient and family needs.
• Deliver interventions that promote comfort, dignity, and quality of life.

What You'll Do
