Job summary

Day City Hospice in Dayton, OH seeks an RN Admissions Nurse to guide patients and families through the hospice admission process. You will perform pre-admission evaluations, educate families about services, obtain informed consent, and collaborate with the interdisciplinary team to ensure comfort and quality of life.

Four 10-hour weekday shifts, including Mondays and Fridays, with a supportive, patient-centered culture. OH RN license and hospice experience preferred.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current RN license in the state of Ohio.
• Current CPR certification.
• Hospice experience preferred (1 year preferred).
• Strong clinical judgment and ability to make independent decisions.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct pre-admission evaluations to determine hospice eligibility.
• Educate patients and families about hospice care and services.
• Facilitate admission process and obtain informed consent.
• Perform comprehensive nursing assessments.
• Develop initial plans of care based on patient and family needs.
• Deliver interventions to promote comfort, dignity, and quality of life.
• Collaborate with interdisciplinary team to ensure exceptional patient care.
