Job summary

Ascend Hospice & Palliative Care MA in Worcester is seeking an Intake Coordinator to manage initial inquiries and move referrals toward admissions. You will forward forms to the Hospice Nurse and Director of Clinical Services, and support families through the admissions process.

The role requires 5–8 years of related experience, an Associates Degree, a valid driver's license with reliable transportation, and the ability to travel.

• 5–8 years of experience in a related capacity.
• Must have a Valid Driver's License and reliable transportation.
• Willingness to travel.

Responsibilities

• Completes initial inquiry forms for all potential admissions and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator.
• Insures that whenever possible, all referrals are converted to admissions.
• If referrals do not convert to admissions, the Administrator will be notified.
• Track all inquiries and referrals daily. This report will be given to the Administrator weekly.
• Assists patients, families and responsible parties in completing the admissions packet as needed.
• Works collaboratively with appropriate staff to insure written or verbal consents are obtained.
