Job summary

AccentCare, Inc. is seeking an Admissions Manager, Hospice in Norwood, MA.

This full-time role directs patient referrals, coordinates with staff, and ensures timely determination of service needs while building relationships with providers and families. The ideal candidate will have an Associates Degree (preferred), hospice/home health or case management experience, at least 1 year of supervisory experience, and strong written and verbal communication skills.

• Ability to work closely with a wide variety of stakeholders.
• 1+ years of supervisory experience or AccentCare tenure.

Responsibilities

• Ensure all patient referrals are received in a timely and complete manner.
• Coordinate with staff to determine initial service needs for patients and families.
• Supervise communication with providers, attending physicians, and hospice staff to coordinate initiation of services.
• Maintain regular communication with the Director-Business Development (DBD).
• Maintain referral statistics and provide monthly reports to DBD.
