Job summary

Vitality Hospice is seeking an Intake Coordinator to serve as the primary point of contact for incoming hospice referrals and to coordinate the intake process from referral through admission. This role requires excellent communication, organization, and attention to detail while providing compassionate service in a HIPAA-compliant environment.

Qualifications

• High school diploma or GED required; Associate's or Bachelor's degree preferred.
• Excellent communication, organizational, and multitasking skills.
• Compassionate, dependable, and committed to providing exceptional customer service.

Responsibilities

• Receive and process incoming referrals via phone, fax, email, and electronic referral platforms.
• Communicate with hospitals, physician offices, skilled nursing facilities, case managers, and other referral sources to obtain required documentation.
• Collect, verify, and accurately enter patient demographics, insurance information, physician orders, and clinical documentation into the EMR system.
• Coordinate with the clinical and admissions team to ensure timely patient admissions and scheduling.
• Verify insurance eligibility and ensure all required documentation is complete prior to admission.
• Maintain accurate referral records and monitor the status of pending referrals.
• Communicate professionally and compassionately with patients, families, referral sources, and internal staff.
• Maintain confidentiality and comply with HIPAA and all company policies.
• Assist with administrative duties related to the intake process.
• Collaborate with clinical leadership to ensure an efficient, patient-focused admissions experience.
