Qualifications

• Excellent written and verbal communication skills
• 1+ years of supervisory experience or AccentCare tenure
• Ability to work closely with a wide variety of stakeholders
• Hospice, home health, or case management experience

Responsibilities

• Ensure all patient referrals are received in a timely and complete manner
• Coordinate with staff to determine patient and family service needs
• Supervise communication with providers, physicians, and hospice staff
• Maintain referral statistics and provide monthly reports to DBD
• Coordinate with Admissions Department and DBD to ensure timely admissions services
• Maintain confidentiality of sensitive information
• Participate in hospice orientation and ongoing education programs
