Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Admissions Coordinator to manage patient referrals, coordinate admissions, and enter data for new patients in a local, non‑remote role. Schedule and communicate with physicians, families, and the care team to ensure timely intake and smooth handoffs.

The ideal candidate has 2–3 years of related healthcare experience, strong communication skills, and an understanding of hospice services.

Qualifications

• High school diploma or GED required.
• LPN preferred. Bachelor's degree preferred.
• Experience communicating with physicians, nurses and other healthcare personnel is highly preferred.

Responsibilities

• Receives and processes referrals from physicians, patients, families, friends, and community.
• Contacts physicians to secure an order for admission.
• Schedules appointments for informed consents and RN assessment visits.
• Explains hospice services and benefits to patients and families as needed.
• Ensures proper notification of admission to the IDT and ancillary providers.
