Responsibilities

• Maintain patient medical records and documentation.
• Verify benefits and obtain insurance authorizations.
• Answer and route incoming phone calls.
• Enter and update patient information accurately.
• Receiving and reviewing hospice referrals.
• Speak with patients and families to gather information and explain services.
• Coordinate with physicians and referral partners.
• Confirm eligibility documentation and required orders.
• Schedule admissions and coordinating clinician assignments.
• Support smooth transitions from hospital or facility to home.
• Identify and resolve barriers prior to admission.
• Maintaining accurate and timely documentation.
