Providence at Home with Compassus in Medford, OR, seeks a Hospice Admissions Coordinator to process patient referrals, coordinate admissions, and enter admission data after all hospice requirements are met.

You will liaise with physicians, patients, families, and care teams; secure orders, coordinate pre-certifications, schedule RN assessments, and ensure accurate charting and documentation while promoting hospice services.

Qualificações

• High school diploma or GED required.
• Bachelor's degree preferred.
• Two to three years of related experience strongly preferred.
• Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsabilidades

• Process patient referrals from physicians, patients, families, and community.
• Coordinate admission of patients and enter admission data.
• Contact physicians to secure orders for admission; arrange nurse if needed.
• Schedule appointments for informed consents and RN assessments.
• Explain hospice services and benefits to patients and families.
• Enter admission data into third-party system and finalize charting.
• Market referral sources and promote hospice services.
