Job summary

Compassus is seeking a Hospice Admissions Coordinator to manage patient referrals, coordinate admissions, and ensure accurate data entry in our systems. You will contact physicians for orders, verify insurance pre-certification, schedule assessments, and explain hospice benefits to families.

You will also market hospice services to referral sources and keep the care team informed throughout the admission process.

Qualifications

• High school diploma or GED required.
• Bachelor's degree preferred.
• Two to three years of related experience and/or training strongly preferred.
• Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement preferred.

Responsibilities

• Receive and process referrals from physicians, patients, families, friends, and community.
• Contact physicians to secure an order for admission.
• Schedule appointments for informed consents and the RN assessment visit.
• Enter admission data into a third-party system.
• Notify the interdisciplinary team (IDT) and ancillary providers of admissions.
• Markets referral sources and build relationships with staff.
• Provide back-up support for the Hospice Care Consultant.
• Explain hospice services and benefits to patients and families.
