Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Admissions Care Partner for on-site, full-time work in West Palm Beach. The role follows a Tuesday through Saturday schedule, 8:30 a.m.–5:00 p.m., managing admissions and coordinating with affiliated hospitals across North Palm Beach County.

Responsibilities include processing referrals, obtaining consents, and supporting patients and families through the continuum of care from intake to field.

Qualifications

• Bachelor’s Degree preferred (in health related or human services)
• 3–5 years of experience in customer service/referral representative in healthcare or community settings
• Strong understanding of medical terminology; hospice experience desirable
• Able to communicate physical and psychosocial needs of patients and families; organized and able to work under deadline

Responsibilities

• Present hospice program and continuum of care to families/patients and facilitate admissions.
• Process referrals, calls, obtain signed consents, complete non-nursing documentation in a healthcare setting.
• Assist admissions from Intake to the Field and respond to patient needs reflecting Trustbridge values.
• Act as a problem solver and facilitator to provide continuity of care for patients and families.
• Coordinate plan of care and treatment (in person or by phone) for hospice-appropriate patients and families.
• Articulate all components of the continuum of care to guide patients to needed services.
