Job summary

VNA Health in Solvang, CA is seeking an Admissions Registered Nurse to initiate and conduct hospice patient admissions, assess patients and families, and plan care for seamless transition to ongoing services. The RN coordinates with the RN Case Manager to ensure timely patient hand-offs.

Candidates must hold a California RN license with at least one year of acute care experience; home health or hospice exposure is preferred.

Qualifications

• Current and active California RN license.
• Minimum one year acute care experience required.
• One year recent home health or hospice experience preferred.
• Bilingual – Spanish preferred, but not necessary.
• Current CPR certification.
• Licensed driver with insured automobile - in accordance with state and/or organization requirements.

Responsibilities

• Performs hospice admissions and initial patient assessment.
• Develops plan of care in collaboration with patient/family and assigns to RN Case Manager.
• Documents patient condition and communicates with physician and care team.
• Provides patient and family education, including self-care techniques and meds/instructions.
• Ensures safe environment and continuity of care, including hand-offs to care team.
