Job summary

VNA Health seeks an Admissions Registered Nurse in Solvang, CA to lead hospice admissions with a focus on community health. The RN assesses patient needs, plans care, and collaborates with the Interdisciplinary Team to ensure seamless transitions from admission to ongoing case management.

The role requires a CA RN license, acute care experience, and CPR certification; Spanish-speaking skills are a plus. This is a full-time, on-site position offering comprehensive benefits and PTO.

Qualifications

• Current and active California RN license.
• Minimum one year acute care experience.
• One year recent home health or hospice experience preferred.
• Bilingual – Spanish preferred, but not necessary.
• Current CPR certification.
• Licensed driver with insured automobile - in accordance with state and/or organization requirements.

Responsibilities

• Provides accurate, efficient admissions to hospice patients.
• Performs initial patient and family assessment and determines care needs.
• Develops and communicates plan of care with patient/family and care team.
• Delivers patient and family education and health instructions.
• Ensures safe environment and maintains accurate clinical notes.
• Interacts with physicians and other team members to coordinate care.
