Job summary

Community Hospice & Health Services (CHHS) is seeking an RN Admission Nurse to join the Patient Care Services team in Modesto, CA. The role covers initial admissions, consent, and planning for hospice services, with collaboration across the hospice care team.

The ideal candidate has a CA RN license, at least one year of nursing experience, and CPR certification; CHPN is preferred. The position supports home-based care and requires strong communication and documentation skills.

Qualifications

• Graduation from an accredited school of nursing.
• At least one year of current nursing experience (LVN or RN).
• Valid California RN license.
• Current California driver’s license and automobile insurance required.
• CHPN certification preferred.
• CPR certification required.”

Responsibilities

• Provide informed consent for hospice services, conduct initial nursing assessment and care planning.
• Evaluate patient condition and history to determine appropriateness for hospice services.
• Triage calls/visits when admissions are not scheduled.
• Maintain communication with the Patient Care Manager and team about patient/family issues.
• Coordinate with attending physicians and other hospice team members to optimize care.
• Document initial assessment and plan of care accurately and promptly.
• Perform home visits to educate caregivers on pain management and symptom control.
• Support patients/families emotionally and spiritually, including at time of death.
• Participate in orientation, staff meetings, and required trainings.
• Maintain confidentiality and HIPAA compliance.

Education

Certified Hospice and Palliative Nurse (CHPN) certification
