Job summary

Seaport Hospice seeks a compassionate Admission Registered Nurse (RN) to assess patients, coordinate hospice services, and guide families through the transition into hospice care. The admissions RN will perform comprehensive assessments, develop individualized care plans, and ensure smooth transitions in the home setting.

The role requires a California RN license with at least 2 years of hospice or related experience, strong assessment and communication skills, CPR/BLS, and reliable

Qualifications

• Current, unrestricted California RN license.
• Minimum 2 years RN experience; hospice/home health/palliative care preferred.
• Must have CPR/BLS certification and valid driver's license.

Responsibilities

• Complete comprehensive hospice admission assessments.
• Develop and initiate individualized hospice care plans.
• Provide skilled nursing care, symptom management, and education.
• Collaborate with physicians, interdisciplinary teams, caregivers, and community resources.
• Educate patients and families on disease progression, medications, comfort care, and self-care techniques.
• Monitor and document patient status, interventions, and outcomes in a timely and accurate manner.
