Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Admission Registered Nurse to assess patients, determine hospice eligibility, and coordinate initial plans of care in a home setting across Ohio. The role emphasizes compassionate, patient- and family-centered care with collaboration from the Inter-Disciplinary Team.

Qualifications

RN license in the state you work
Two years of experience as a registered nurse, hospice experience preferred
Current driver’s license
Current CPR and BCLS certification

Responsibilities

Enable patients to spend quality time with loved ones and engage in activities they enjoy.
Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers
Ensure all patient’s initial needs are addressed
Collaborate with the Inter-Disciplinary Team regarding initial plan of care, patient’s condition, and other essential information
