Job summary

OhioHealth at Home in partnership with Compassus seeks a Hospice Clinical Liaison to promote Compassus values and support referral sources by matching patient needs with hospice services. You will assist with admissions, coordinate with hospitals and facilities, and ensure proper documentation is completed for patient eligibility.

The role requires an RN license, a nursing degree is highly preferred, and 2–3 years of related experience in healthcare/hospice settings.

Qualifications

• RN License required
• Bachelor's degree in Nursing highly preferred
• Two to three years of related experience required
• Hospital and/or long-term care clinical experience highly preferred
• Experience in healthcare marketing preferred

Responsibilities

• Attends and participates in staff meetings, educational programs, and community events.
• Explains hospice services and benefits to patients, family members, and significant others.
• Assists with admissions and ensures forms are completed and submitted to the Admission Coordinator.
• Assesses referred patients' clinical needs and collaborates with physicians to determine eligibility.
• Facilitates patient discharge from hospital to home and coordinates supplies and services.
• Oversees GIP patient admission process based on symptom management needs.
• Builds relationships with facilities and hospitals to promote admissions and growth.
