Job summary

Ohio’s Hospice in Newark, OH is seeking a compassionate Admission Care Liaison (Registered Nurse) for full-time direct patient care. The role focuses on hospice admissions, patient assessments, and care coordination across hospital and community settings.

You will collaborate with physicians and teams to ensure smooth transitions, educate families, and support personalized plans of care. A driver’s license and Ohio RN license are required.

Qualifications

• Graduate of an accredited nursing school required.
• Current RN license in Ohio with no board actions.
• Two years of acute care nursing experience preferred.
• CHPN certification preferred; available for preparation and support.
• Current CPR certification required.
• Computer skills to document services and patient care.
• Valid driver’s license and acceptable driving history.
• Ability to travel as needed in varying weather and road conditions.
• Ability to pass background check and drug screening.

Responsibilities

• Conduct comprehensive patient and family assessments addressing physical, emotional, psychosocial, and spiritual needs in hospitals, long-term care facilities, and home settings.
• Educate patients and families about hospice services, eligibility, and resources while supporting informed decisions.
• Collaborate with interdisciplinary team to develop individualized plans of care.
• Communicate clinical findings to hospice physicians to support eligibility and certification.
• Provide ongoing assessments, symptom management, and care coordination across care settings.
• Partner with healthcare teams to promote continuity of care and timely discharge planning.
• Assist across departments and support organizational operations and patient care initiatives.
• Participate in quality improvement activities to enhance outcomes and experiences.
• Mentor and orient newly hired staff, promoting teamwork and professional development.
