Job summary

Reserve Care seeks a Clinical Admissions Liaison-Registered Nurse (RN) in Ashtabula, OH. The role covers admissions and information sharing for hospice patients in homes or facilities, working closely with referrals and physicians.

The nurse provides nursing care, education, and emotional support, documents accurately, and maintains HIPAA compliance. A BSN is preferred, with Ohio RN licensure and a valid driver’s license required.

Qualifications

• Ohio RN license in good standing.
• Minimum of 1 year nursing experience in intake, home care, hospice, or oncology.
• Valid driver’s license with reliable vehicle.

Responsibilities

• Perform information assessment and admission services in patients' homes or care facilities.
• Coordinate with Referral Department and physicians to ensure appropriate referrals.
• Document thoroughly and timely in electronic medical records.
• Maintain HIPAA privacy standards and professional boundaries.
• Collaborate with the IDT to coordinate patient care and resources.
