Job summary

The VIOS Clinic is seeking an experienced Hospice Admissions Registered Nurse (RN) in North Carolina. The role involves evaluating and admitting patients, creating individualized care plans, and coordinating patient services in home settings.

You will perform comprehensive assessments, document at the point of service, and work independently with strong communication, organization, and time-management skills.

Qualifications

• Current RN licensure in North Carolina.
• Minimum of 1 year of clinical nursing experience.
• Strong assessment and documentation skills.
• Ability to work in a home setting.
• CPR certification; valid driver’s license; 12 hours annual CE.

Responsibilities

• Evaluate and admit hospice patients; develop individualized care plans; coordinate patient services.
• Perform comprehensive assessments and document care at the point of service.
• Work independently with strong communication, organizational and time management skills.
• Participate in on-call rotations and staff meetings; travel as assigned.
• Deliver compassionate, dignified care to hospice patients with effective pain management and quality of life.
