Job summary

VIA Health Partners in Davidson, NC is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to deliver compassionate hospice and palliative care in home or facility settings. You will collaborate with an interdisciplinary team to implement and monitor care plans, educate patients and families, and ensure comfort and dignity at end of life.

The role requires a nursing license, multi-state compact preferred, CPR certification, minimum 2 years in life-limiting illness

Qualifications

• Diploma or associate degree in nursing; bachelor’s preferred.
• Active, unrestricted RN/LPN license; multi-state compact preferred.
• Current CPR; CHPN preferred.

Responsibilities

• Deliver patient care in home or facility settings per plan of care.
• Monitor patient status; communicate changes to care team.
• Educate patients and families about palliative care and treatment options.
• Coordinate with social worker and interdisciplinary team to arrange resources.
• Apply infection control principles and maintain thorough documentation.
