Job summary

Advocate Health seeks a part-time Hospice RN to oversee admission processes and coordinate interdisciplinary care for patients in need of hospice services.

You will assess patient history, perform physical exams, determine eligibility and collaborate with physicians and the IDT to tailor plans of care. Travel within the service area is required as part of providing comprehensive hospice support.

Qualifications

• RN license issued by the state of practice.
• BLS for Healthcare Providers certification (AHA) within 6 months if not already held.
• Valid driver’s license issued by the DMV.

Responsibilities

• Responsible for admitting patients to hospice services.
• Leads the interdisciplinary care team for each patient and family.
• Obtains patient history and completes a physical assessment.
• Assesses hospice eligibility and level of care with the attending physician and IDT.
• Meets patients and families to explain hospice services and obtain consent.
• Verifies verbal prognosis with medical director and IDT.
• Reviews Medicare/insurance eligibility and documents data for billing.
• Develops the plan of care with patient and family involvement.
• Coordinates services and collaborates with IDT for consistent care.
• Arranges equipment and supplies needed for care delivery.
• Communicates with IDT for handoffs and care planning.
• Provides hospice education to hospital staff when directed by management.
• Serves as an expert resource for hospice and palliative care services.
