Job summary

PruittHealth Hospice Macon is seeking a dedicated Hospice Admissions Nurse (RN) to join our team in Georgia. The role supports patients in homes or care facilities, ensuring smooth admissions and comprehensive care planning under Medicare and hospice guidelines.

The successful candidate holds an active RN license, demonstrates compassionate patient care, and can document and coordinate care effectively in a fast-paced hospice environment.

Qualifications

• Current, active Registered Nurse (RN) licensure in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
