Job summary

PruittHealth Hospice Greenville is seeking a Registered Nurse Case Manager to coordinate patient care in home or facility settings in Greenville, SC. You will perform assessments, develop care plans, and ensure documentation aligns with policy and individualized care plans.

Qualifications

• Current, active RN licensure in the state of practice.
• Hospice or home health experience preferred.

• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
