Qualifications

• Education: Bachelor of Science in Social Work.
• Licensure/Certification: BLS.
• Experience: 2 years of social work in health care; 1 year in palliative care or hospice.
• Driver's License: Yes; to drive Augusta Health vehicles per policy.

Responsibilities

• Coordinates hospice referrals, admissions, and transitions of care.
• Communicates with hospital staff, hospice physicians, and other involved physicians.
• Assesses patients for hospice eligibility and identifies needs.
• Documents care and ensures timely submission of paperwork.
• Educates hospital staff on hospice benefits and coordinates care across settings.
